        - Instrument and/or instrument systems are defined by type (product name)
          and not quantity of sets. It is suggested that a minimum of 4 sets of each
          implant and instrument system are made available for the workshops to
          ensure availability and accessibility to all course registrants.

        - Each additional implant and/or instrument set beyond the allotted number will
          have a fee of $1,000.

        - Each additional representative beyond the allotted number will be charged a
           registration fee of $250.

        - representatives are responsible for providing their own scrubs
